1. You could still have Donna trapped in the Stepford!AU, but have her take some initiative in getting herself (and everyone else) out of it. It would give her something to do other than be a victim--lets see some of that Donna Noble resourcefulness and why the Doctor needs it. And it could still be a wistful life-that-might-have-been, but I also want to see her rejecting it and choosing the life of adventure that she, y'know, has chosen. That process would show us some character development and give her some self-realization--we still haven't gotten that much detail about why Donna Noble specifically wants travel and adventure and making a difference over a "normal life." She could even still fall for the imaginary husband and seek him in the real world, but with the realization that she wants more than the 2.5 kids and house in the suburbs. Basically, Donna should've been more active and her character should've been explored more.
2. You could still have River Song be the Doctor's future!love (or whatever) but do it in a subtle way. Don't have her constantly pointing out how much she knows (but refuses to tell) and explaining every five seconds how she knows the Doctor better than ANYONE ELSE EVER and how she's SOOO important. It's just irritating, having the character make sure everyone knows how much she knows and matters but refusing to say.
Instead, have her slip up and inadvertently reveal knowledge that only someone incredibly important to the Doctor could have known. Make her emotions apparent in a subtle way, so that she doesn't have to go bashing everyone over the head with who she is, but rather, so that it slowly becomes apparent to the audience. This way we'd go along for the ride, realize that there's a mystery to her and want her to tell us more, instead of just wishing she'd shut the hell up already about how important she is.
3. You could even still have River end up in the magical afterlife (everyone "lives," yay). But show her reading a book or having an imaginary adventure or something. Not taking care of kids, because, y'know, she's a woman and the most fulfilling afterlife would obviously be taking care of a child forever (ew ew ew). Something more interesting than the heteronormative default, please!
